IMPORTANT INFORMATION: Please read carefully and check the box indicating your understanding.

I, the undersigned, upon joining the Heritage Center of Jewish Community Center of Greater Kansas City (The J), I agree to abide by the policies of the organization as well as all rules and regulations in all areas, activities and programs.

I understand this non-refundable, non-transferable membership is for one year from the first of the month nearest to the date joined.

I understand that I may be required to present a membership ID card when entering or using the The J fitness facilities during authorized Heritage Center times. I understand that use of the fitness facilities other than the authorized times is not permitted.

I understand that my Heritage Center membership does not give me member pricing on any programs other than those sponsored directly by the Heritage Center.

The J reserves the right to photograph or videotape members and guests and use these materials for marketing purposes, including, but not limited to, insertion on The J website, J social networking, program guide and other promotional materials.

I understand that by signing this membership form I authorize The J to utilize information collected on this form for reporting to funders, direct communications and marketing purposes.

I understand that participation in programs and use of recreational and workout facilities involves the risk of accidental injury despite all safety precautions. I acknowledge that I use all equipment and facilities at my own risk.

By signing this form, I acknowledge that I am aware of the potential risks of participating in activities and/or programs at The J and agree to hold harmless the management, agents or employees of The J liable for any injury that I or members of my family might sustain.

IMPORTANT: RELEASE AND WAIVER OF LIABILITY AND INDEMNITY

You hereby acknowledge and agree that use by Member and/or by Member’s spouse and/or by minor children of The Jewish Community Center of Greater Kansas City (The J) facilities, services, equipment, or premises, involves risks of injury or illness to persons and property, including those described below, and Member assumes full responsibility for such risks. Risk of injury or illness includes (but is not limited to): injuries arising from use by Member or others of exercise equipment and machines; injuries arising from participation by Member or others in supervised or unsupervised activities or programs at The J; injuries and medical disorders arising from exercising at The J such as heart attacks, strokes, heat stress, sprains, broken bones, and torn muscles and ligaments, among others; and accidental injuries occurring anywhere in The J dressing rooms, showers and other facilities; illness including any airborne or contact colds, flus or diseases. Member further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the State of Kansas and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full force and effect. Member has read this release and waiver of liability and indemnity clause, and agrees that no oral representations, statements, or inducement apart from this Agreement have been made.
